This is a great write-up! I would add another major knock against organoids to the list of (lacking) vasculature, immune system, and aging: At the risk of stating the obvious, there are no other organs to interact with! No single organ, whether it is a kidney or the pancreas, exists in a vacuum. There is constant endocrine and other cross-talk between tissues.

Something as simple as Vitamin D gets absorbed in the gut and multiple sequentially dependent hydroxylation steps in the liver and kidneys. How can you model this in an organoid? The answer is you cannot.

These examples are nearly limitless. Here are just a few more:

- Red blood cells are produced in the bone marrow, but require EPO from renal interstitial cells, iron absorbed from the gut (under the influence of the liver peptide hormone hepcidin), along with a host of other cytokines, B vitamins and compounds like copper that all have their own multisystem handling

- The hypothalamus, pituitary, and adrenal glands (the “HPA axis”) control virtually all metabolic processes in the body and tightly regulate levels of cortisol and other hormones minute to minute

- Even if you could introduce a perfectly recreated vascular system to an organoid (and you can’t), it would still not reflect a living being because there would be no heart and lungs to adjust vascular tone, dissolved O2 and CO2 gases, and a lot of other variables

The term “irreducible complexity” has a lot of baggage due to its association with “intelligent design” (aka neo-Creationism). However, I think we should all be able to agree that organoids are overly reductive, and could never adequately mimic mammalian physiology closely.

The argument “most drugs fail in animals” should not be an endorsement of the cell culture or organoid approach, but a call to humility about how unbelievably complex all of these systems really are.

>Matrigel is a gelatinous protein mixture secreted by Engelbreth-Holm-Swarm mouse sarcoma cells. Yes, cancer cells. And as you would expect would be the case with anything secreted by cancer cells, it is both extremely complicated and highly heterogeneous.

It gets even worse. Matrigel is produced by injecting the cancer cells into mice, waiting for the mice to grow tumors, then killing the mice and grinding up the tumors. It's rather barbaric.
